In 2012, the ambulatory care services team at Saint Joseph's began working to achieve NCQA PCMH Recognition for its Family Health Center. An interdisciplinary team of professionals was brought together to accomplish this goal. While they seemed to be making progress toward establishing a PCMH, the team recognized that its efforts to achieve NCQA PCMH Recognition were stalled and that true transformation of the practice was not taking place.
