Keeping children healthy is the primary goal of Stony Brook Children’s Service primary care program. Five practices located throughout Suffolk County provide comprehensive primary care, including routine well and sick care for children from birth through age 21.

To meet its goal of keeping patients at the center of everything it does, in 2012 Theresa Lang Children’s Center embarked on the journey to create a PCMH. With grant funding from the DOH H-MH Demonstration Project, the NYHQ Community Medicine Department focused on strengthening patient care and better managing patients’ overall health at its pediatric center.
